Attach a detailed description of modification/improvement, including:

• Location
• Size
• Color
• Material
• Contractor, if any
• COPY OF PHYSICAL PROPERTY SURVEY (SHOWING EASEMENTS, PROPERTY DIMENSIONS, BUILDING SETBACKS, ETC.)
• Plans or drawing of structure to be erected, placed or maintained on property
• Narrative description of specific improvements (including colors to be used)
